    • APR Definition: Percent of all children exiting Part C who received timely transition planning to support the child’s transition to preschool and other appropriate community services by their third birthday including IFSPs with transition steps and services.
    • Calculation: [(# of children exiting Part C who have an IFSP with transition steps and services) divided by the (# of children exiting Part C)] times 100

  • C–8B
    • APR Definition: Percent of all children exiting Part C who received timely transition planning to support the child’s transition to preschool and other appropriate community services by their third birthday including: notification to LEA, if child potentially eligible for Part B.
    • Calculation: [(# of children exiting Part C and potentially eligible for Part B where the notification to the LEA occurred) divided by the (# of children exiting Part C who were potentially eligible for Part B)] times 100

  • C–8C
    • APR Definition: Percent of all children exiting Part C who received timely transition planning to support the child’s transition to preschool and other appropriate community services by their third birthday including: transition conference, if child potentially eligible for Part B. (20 U.S.C. 1416(a)(3)(B) and 1442)
    • Calculation: [(# of children exiting Part C and potentially eligible for Part B where the transition conference occurred) divided by the (# of children exiting Part C who were potentially eligible for Part B)] times 100

  • Data Collection Notes: Data were collected in MEGS through a local self-assessment. Local early intervention programs selected a random representative sample of children. Local early in-tervention programs were required to sample 10% of their snapshot number or a minimum of ten records, whichever is greater. The records were a sample chosen from July 1, 2010 through June 30, 2011.
